Dear Customer,<br />
Following up on our earlier negotiations alert, we’re writing to inform you that effective midnight August 30, 2011 the following providers are no longer part of the Blue Shield of California network: <strong>Lucile Salter Packard Children’s Hospital (LPCH) and Lucile Packard Children’s Hospital Medical Group (LPMG)</strong>, located in Santa Clara County.<br />
The impact of this termination is summarized below. Medicare members are <span style="text-decoration: underline;">not</span> impacted by either termination.</p>

<h1>HMO Facility Exception<span class="p">- Palo Alto Medical Foundation [PAMF] members under age of 26 will have </span>continued access <span class="p">as this is their primary hospital.</span></h1>
<p>Rest assured that this will not alter your employees’ benefits or interrupt their access to quality care. Blue Shield of California and Blue Shield of California Life &amp; Health Insurance Company continue to have an extensive network of physicians and hospitals throughout the San Francisco Bay Area that are available to serve members who are currently receiving care from this hospital and medical group.</p>
<p class="s5">Reason for Termination</p>
<p>Blue Shield has been negotiating with Lucile Packard Children’s Hospital (LPCH) and Lucile Packard Medical Group (LPMG) since February 2011 in the hopes of reaching a mutually agreeable contract that would allow these providers to remain in our network. One of the main factors in the negotiation was the fact that Lucile Packard was already one of the most highly compensated hospitals in the Blue Shield network prior to their requested rate increases for the new 2-year contract term. After months of negotiations, Blue Shield and Lucile Packard were able to reach a verbal agreement on reasonable increases over the next two years for both the hospital and the medical group, and Blue Shield was hopeful of signing a new contract this month.<br />
Unfortunately, despite agreeing on rates, the contract has now terminated due to Lucile Packard’s introduction of a new requirement into the negotiation, to include a future entity in the contract. The providers want us to agree to extend the contract rates to a new entity which they plan to open in approximately one year. We value our relationship with Lucile Packard, however we are unable to accept these terms because we can’t contract with</p>

an entity not yet incorporated. This request by the provider would also impede our ability to provide our members in this area with access to quality health care at an affordable price, and would not be in our members’ best interest.<br />
Consistent with our not-for-profit mission of providing access to quality health care at an affordable price, Blue Shield seeks financial arrangements with providers that are sufficient to cover their cost of providing health care, provide them with a fair margin and enable us to offer competitively priced coverage to employers and members.  These contractual changes would have significantly driven up costs for employers and members.</p>
<p class="s5">Impact to transplant services</p>
<p>Blue Shield’s contracts for the transplant programs at both Lucille Packard Children&#8217;s Hospital and Stanford Hospital (both in Palo Alto) terminated on 8/31/11. The transplant amendment is a 3- party document and tied to our general contract with Lucille Packard Children&#8217;s Hospital which has terminated.<br />
Members currently or previously receiving transplant services from Lucile Packard or Stanford will continue their course of treatment at the appropriate facility. In the event of imminent termination, the Transplant Case Management team will contact all transplant patients assuring the members they will not be affected by the termination.</p>
<p class="s5">Alternative Providers</p>

<p class="s5">Continuity of Care</p>
<p>Impacted members who are currently receiving care from the hospital or a medical group physician, or have a procedure scheduled within 180 days of the termination date at the medical center, may request continuity of care (COC). If COC is approved, the member will receive the preferred (network) benefit level.  Members within the above<br />
circumstances should call our customer service line at (800) 200-3242 to make such requests.<br />
Impacted members not approved for Continuity of Care may continue to utilize LPCH and LPMG providers if they wish. However, doing so means that reimbursement for services rendered will be at the non-preferred (non-network) provider amount, effective August 31.</p>

<td>In California, most minor traffic offense convictions will remain on your record for 3 years from the violation date and count as 1 point.More serious traffic offenses, such as hit and run and reckless driving will remain on your record for 7 years from the violation date and count as 2 points.</p>
<p>Accidents are reported for 3 years from the accident date. If you are found to be at fault, the accident normally counts as 1 point.</p>
<p>Since January 1, 2007 in California any DUI offense on your record will appear for 10 years from the violation date. Before 2007 DUIs were kept on a California driver&#8217;s record for only 7 years but now out-of-house / public driving record printouts continue to show the DUI offense for 10 years.</p>
<p>In 2007 new CA legislation extended the reporting period for DUI offenses from 7 to 10 years for all public requestors, including insurance companies. The law allows insurance companies access to the driving record information to properly apply the new provisions of the Insurance Code established under Senate Bill 597 (2005), to determine a customer’s eligibility for a good driver discount. Based on these laws, drivers with a DUI violation occurring within the past 10 years are not entitled to receive a good driver discount.</p>
<p>Actions taken against your driving privilege, such as a suspension or revocation due to a DUI or a failure to provide proof of financial responsibility, will be reported for 3 years from the proof termination date or the reinstatement date, whichever is earlier.</p>
<p>A Failure To Appear for DUI offenses will be reported for 10 years from the violation date. All other Failure To Appears and Failure to Pay fines will be reported for 5 years from the violation date.</p>

<h2>Who can be an applicant?</h2>
<p>Any person of any age that meets the PCIP Eligibility requirements. The following individuals can be an applicant:</p>
<ul>
<li>Person 18 years of age or older who are applying for coverage himself/herself;</li>
<li>Parents (natural or adoptive);</li>
<li>Legal Guardians;</li>
<li>Stepparents;</li>
<li>Foster parents;</li>
<li>Caretaker relatives for a minor;</li>
<li>Minor not living with persons listed above (i.e. emancipated minor)</li>
</ul>
<p>However, each individual applying for the PCIP must complete their own application, because the PCIP does not offer dependant coverage.</p>
<h2>What documentation must I send with my PCIP Supplemental Application?</h2>
<p>Your PCIP Supplemental Application must contain the following:</p>
<ul>
<li>A personal check, cashier&#8217;s check or money order for one month’s premium payable to the Managed Risk Medical Insurance Board (MRMIB) in the amount for the program you prefer on the PCIP Supplemental Application, question #13.</li>
</ul>
<p>Your monthly are based on your age and where you live. Your monthly are based on your age, where you live, and whom you are applying for. (I.e. just applying for yourself &#8211; PCIP, yourself plus one dependent &#8211; MRMIP).</p>

<li>Proof of pre-existing condition as shown by:

<li>Copy of letter indicating individual coverage has been offered in excess of the MRMIP preferred provider organization (PPO) monthly premiums, in the geographic region where you reside. See MRMIP PPO monthly premiums pages 16 – 21 of the MRMIP Handbook.</li>
<li>Proof of U.S. Citizen/U.S. National as shown by:
<ul>
<li>U.S. Passport;</li>
<li>Birth certificate;</li>
<li>Naturalization/Citizenship certificate;</li>
<li>American Indian or Alaska Native enrollment document from a federally recognized tribe;</li>
<li>A Certificate of Degree of Indian Blood (CDIB) from the Bureau of Indian Affairs; <strong>or</strong></li>

<li>As of January 1, 2006, all insurance companies are required to report insurance status information to DMV for all private use vehicles (CVC §16058).</li>
<li>As of July 1, 2006, law enforcement and court personnel have access to DMV records to verify that your California registered vehicle is currently insured (CVC §16058.1).</li>
<li>Effective October 1, 2006, your vehicle registration is subject to suspension if the liability insurance is cancelled, OR if your insurance company has not electronically provided evidence of insurance when you purchase and register your vehicle, OR if you provide DMV with false insurance information (CVC §4000.38).</li>
</ul>
<h4>What does this mean to the California vehicle owner?</h4>
<ul>
<li>You will have greater assurance that the vehicle next to you on the road is covered by liability insurance.</li>
<li>You will have the opportunity to renew your registration online, over the phone, or at a self-service terminal.</li>
<li>You will not have to submit paper proof of insurance with your private-use vehicle registration renewal.</li>

<p>Your auto insurance policy consists of sections that define every type of auto insurance coverage offered by your company. Policies are distinguished by the state in which they&#8217;re issued, and coverages available may vary among states. Your policy simply explains the types of coverage available and how or when they can be used. For your specific policy information, please refer to the terms, conditions, limitations and exclusions contained in your specific policy.</p>
<ul>
<li><strong>Liability</strong> – This coverage consists of Bodily Injury and Property Damage (BI/PD), which covers your legal liability, up to the limit you select, for damages caused in a covered vehicle accident. Under BI/PD, you’re paid for damages to an injured person and for property damage that you are legally obligated to pay as a result of an accident. If they cover an accident for which you are sued, they will pay for a lawyer to defend you, too.</li>
<li><strong>Personal Injury Protection (PIP)</strong> – Available in certain states and commonly referred to as &#8220;no-fault insurance,&#8221; PIP covers your medical bills and often lost wages if you are disabled or unable to work as a result of an accident. PIP also usually covers the cost of personal services you must now pay someone else to do for you. PIP coverage is subject to a limit, which is specified in your policy.</li>
<li><strong>Medical Payments (Med Pay)</strong> – This coverage applies no matter who is at fault and covers the cost of reasonable and necessary medical care provided to you as the result of a car accident. The coverage is often limited to a specified time period following the accident (usually three years) and the amount of coverage you chose when you purchased the policy.</li>
<li><strong>Uninsured/Underinsured Motorist (UM/UIM)</strong> – UM/UIM coverage pays for damages that you are legally entitled to recover for your bodily injury. In general, this coverage provides what you would have received from the other person&#8217;s insurance company had that person been insured. UM/UIM may also protect you if the person who caused the damage does not have enough insurance. Uninsured Motorist Property Damage coverage is available in some states and provides protection for damage to property caused by a person without insurance.</li>
<li><strong>Collision</strong> – If your vehicle overturns, or if it collides with another vehicle or object, Collision coverage pays for the damage to your vehicle. Collision involves a deductible amount you select when you purchase your policy. This amount, varies from $100 to as high as $2500, and is the amount you are required to pay in the event a claim exceeds the deductible amount.</li>
<li><strong>Comprehensive</strong> – This coverage pays for damage caused by an event other than a car collision, such as fire, theft, vandalism, and hail or flood damage. Comprehensive also covers damage from an animal hit. Like Collision coverage, a deductible usually applies.</li>
<li><strong>Custom Parts or Equipment (CPE)</strong> – Collision and Comprehensive coverage each provide up to $1,000 of coverage for custom parts or equipment, which are accessories and enhancements permanently installed in or on your vehicle. Parts and equipment offered by the manufacturer or installed by the dealer at the point of sale are not custom parts or equipment, but they are included under your standard Collision and Comprehensive coverage.</li>
<li><strong>Additional Custom Parts or Equipment (ACPE)</strong> – This coverage is available if you have more than $1,000 of custom parts or equipment on your vehicle. If you purchase ACPE, they will pay for custom parts or equipment damaged as a result of a covered incident, up to the amount shown on your Dec Page.</li>
<li><strong>Rental Reimbursement</strong> – If you buy Rental Reimbursement coverage, they will reimburse you for rental car charges incurred while your vehicle is being repaired after a covered accident. You can only buy Rental Reimbursement if you buy Collision and Comprehensive coverage. Rental Reimbursement is limited to 30 days and is subject to the maximum per day amount shown on your Dec Page.</li>
<li><strong>Loan/Lease Payoff</strong> – This coverage protects you when your covered vehicle has been deemed a total loss and you owe a lender more money than the vehicle is worth. If you buy Loan/Lease Payoff and your vehicle is declared a total loss, Loan/Lease Payoff will pay the difference between the vehicle&#8217;s actual cash value and the amount you owe to the lender. However, check the maximum they will pay under this coverage as it can vary by company.</li>
<li><strong>Roadside Assistance</strong> – Roadside Assistance will cover towing to the nearest qualified repair facility.  Some companies also have several other services that are available to you which are included with the Roadside Assistance Coverage.</li>

Collect contact information—especially phone numbers—for drivers, passengers, and witnesses involved in the accident; your insurer might need to speak with them to determine who’s at fault for the accident. If the police responded, or if anyone was injured, your insurer will want to know that, too. Also, get details about the other cars involved, from make/model to color, and take note of whether the cars were damaged and if they are, where the damage occurred, and take pictures from your camera phone.
